Treating Burned Skin & Scars Left Behind

What to do first when treating burne skin
The first priority is to prevent further burning. If the victim is on fire, they should be wrapped in a blanket, heavy coat (or whatever is available) or pushed to the ground to smother the flames. Douse any smouldering clothing with water. Immediately move the person to a safe place away from the fire. If the victim is unconscious, check their airway, breathing and circulation.

Carefully remove any heat retaining clothing and jewellery over the damaged skin to prevent further damage. This same rule applies to scalds.

The next step is to immerse the burn in cool running water for at least 30 minutes to help relieve the pain. If you can't cool the burn immediately, this procedure will still help to some degree up to 2 hours after the injury has occurred.

In very serious cases, the loss of skin cover causes burn victims to significantly drop body temperature. Cooling the victim for a long period of time can endanger their life. It is best to cover the victim with a clean sheet or blanket.

Specific care should be given to electrical and chemical injuries. Even though an electrical burn may appear small in size, it can cause damage in deep tissues or interfere with electrical activity of organs such as the heart and brain. This interference may require CPR at the scene. All electrical injuries should be transported to the hospital.

A chemical injury can damage the skin for a long period of time. If a victim has been exposed to a chemical, remove the clothing immediately and wash the area with copious amounts of water. If the antidote to the offending chemical is known, it should be used instead of water. This process should be continued over a period of time. If the chemical is in the eye, ensure the eye is irrigated with normal saline, if available, or water throughout transport to the hospital.

Do not apply ice to the burn. It compounds the injury by decreasing the blood flow to the burned area, thus starving the damaged tissues of vital fluids and proteins.

How to treat burns
Keep burns adequately covered and free of infection. The burned area should not be wiped clean or have creams or other substances applied. Application of a clean sheet or towel over the injured area is recommended.

Victims who have a burn area that is more than two of their hand spaces in size, an electrical or chemical injury, or someone who has other health problems or received other injuries, should be transported to hospital.

For mild burns which only damage the very top layer of skin and just below, give the burn a good clean and dress the area with a simple plastic layer or even kitchen cling-wrap. By covering the burns with these 'occlusive'-type dressings, the healing process is sped up and it also stops the pain of the burn by sealing off any exposed skin nerve endings that may be 'jangling' and raw from the burn.

Leave a blister alone. It provides a natural cover to keep infection out.

How do burns heal?
If there is minimal damage the normal top layer of skin and there are enough remains of hair follicles and sweat and oil glands, then regeneration of the skin can occur.

When the burn is deep, the skin structure is destroyed and a different type of healing process occurs. Here, other specialized types of cells called Fibroblasts, proliferate and fill the skin defect with scar tissue. This is satisfactory for the short term but this type of tissue is quite stiff and doesn't stretch very well. It also looks very ugly. This causes a problem with important places like the hands where the skin is normally soft. Skin grafting is the best way to manage this type of scar tissue.

The bottom line
A burn injury can be devastating and no burn should be taken lightly. If you have a burn that needs more than the simplest treatment, you should see your doctor for further assessment.
